Kindergarten and Transfer Student Forms
KINDERGARTEN REQUIREMENTS:
Vaccine Record - the following are required:
- Polio Vaccine: 3 doses with the latest one given on or after 4th birthday OR any 4 doses
- DTaP (Diptheria, Tetanus, acellular Pertussis): a total of 4 doses with one of these doses on or after the 4th birthday OR any 5 doses
- Measles, Mumps and Rubella (MMR): 2 doses--one after 1 year of age & one prior to kindergarten. Written proof of a positive MMR titer is accepted in lieu of MMR #2.
- Hepatitis B Vaccine: 3 doses OR written proof of titer
- Varicella (Chicken Pox) Vaccine: 1 dose or written documentation of disease from physician
Physical Examination Form - to be completed by physician (exam must be within 1 year prior to enrollment). Form located to the right on this page.
Dental Form – to be completed by dentist (exam must be within 1 year prior to enrollment). Form located to the right on this page.
Growth and Development Form must be completed by parents/guardian within 30 days of enrollment. From located to the right on this page.
**Parents with a student who has a September birthday should contact their child's doctor about an immunization visit just to receive their required vaccines prior to the start of school. If your student has their physical exam or immunization appointment after the first day of school, the nurse needs written documentation from your child's doctor with the appointment date prior to the start of school.
**Please inform the nurse of any severe allergies or medical conditions prior to your child starting school to ensure the proper forms are completed and medication is in school. REQUIREMENTS FOR TRANSFER STUDENTS:

- DPT Vaccine Series (must have a minimum of 5 doses with the latest one given on or after 4th birthday)
- Polio Vaccine Series (must have a minimum of 4 doses with the latest one given on or after 4th birthday)
- Measles, Mumps and Rubella (must have two doses- one after one year of age & one prior to kindergarten). Written proof of a positive MMR titer is accepted in lieu of MMR #2.
- Hepatitis B Vaccine (completed series-usually 3 doses) or written proof of titer
- Varicella (Chicken Pox) Vaccine or Documentation of Disease
